the community! Responsibilities About the Role During visits that
may last up to one hour, our providers assess a member’s current
health status, medical history, medication adherence, social
environment, and other risks. This approach provides invaluable
insights into a member’s overall health that are often missed
during standard office visits. The Matrix Comprehensive Health
Assessment (CHA) enhances care quality and allows us to address
multiple care gaps in a single visit. Our providers: Perform
assessments for adults and geriatrics, including medical history,
diagnosis, treatment, health education, physician referrals, case
management referrals, follow-up, and thorough documentation in line
with our guidelines and protocols. Administer and order necessary
point-of-care and lab tests. Collaborate with physicians, case
managers, social workers, family members, key caregivers, and other
medical personnel as needed. Work alongside Primary Care Physicians
(PCPs) for patient education and follow-up as required. Provide
services at the member’s residence, which may include their homes
or Skilled Nursing Facilities. Benefits of Working with Us

Assessments are conducted Sunday through Saturday from 8 AM to 7
PM, allowing you to work during the day, evening, or weekend as per
your convenience. Competitive Pay – Your earnings are based on the
number of assessments completed. Increase your income potential by
being available for more visits and traveling to see members
outside your home area. Those averaging 20 to 100 visits monthly
can earn between $1,800 and $11,500 per month, based on a
per-assessment rate of $90 to $115 , depending on your state.

training, you will be assigned a preceptor, receive personalized
feedback, and have free access to continuing education
opportunities. Additional Licensure – If business needs arise, we
will assist you in obtaining additional state licensure and
credentialing in neighboring states or others you may wish to visit
while conducting health assessments. Full-time Opportunities – Our
PRN providers have the option to transition to full-time roles
supporting their home location or a specific state or region, along
with a full-time sign-on bonus. Qualifications Skills & Experience
That Stand Out A Master’s Degree or equivalent experience with
successful completion of NP or PA licensure. Current NP or PA
licensure in the state of practice, including prescription
authority or the ability to obtain it. Board certification from
NCCPA, AANP, ANCC, or AACCN in Adult, Adult Gerontology Primary
Care, Adult Gerontology Acute Care, Family, Emergency, or Acute
Care specialty. Current BLS, ACLS, or CPR certification. Preferred
one year of experience as a Nurse Practitioner or Physician
Assistant; new graduates are encouraged to apply! A valid state
driver’s license and proof of adequate automobile insurance for
your state of residence. Strong computer skills and familiarity
with health/medical record software. Excellent verbal and written
