Job Description:
Agape Care Group is seeking a dedicated Hospice Liaison to join
our compassionate team, providing critical connections between our
services and the patients who need them most. In this role, you
will be integral in facilitating the seamless transition of
patients into hospice care by working closely with healthcare
providers, patients, and their families. This position requires a
deep understanding of hospice care practices, excellent
communication skills, and the ability to sensitively navigate
patient needs during challenging times. Evaluate patient
eligibility for hospice services based on medical criteria and
consultations with healthcare providers. Establish and maintain
strong relationships with referring physicians, hospitals, and
other healthcare facilities to promote our services. Provide
detailed information and support to patients and families about
hospice services, including expectations, goals, and patient care
plans. Coordinate and participate in community outreach programs to
increase awareness and understanding of hospice care. Handle
administrative duties related to patient intake processes,
including accurate documentation and compliance with healthcare
regulations. Strong interpersonal and communication skills Ability
to handle sensitive situations with compassion and professionalism
Deep knowledge of hospice care regulations and eligibility criteria
Proficient in healthcare IT systems Strong organizational and
multitasking skills Medical Insurance Dental Insurance Vision
Insurance 401k Current licensure as a Registered Nurse (RN) or
Licensed Practical Nurse (LPN) in the state of practice
(preferable, depending on state regulations) Certified Hospice and
Palliative Nurse (CHPN) certification is a plus
