Wellness – Join Us as a Community Health Worker (Behavioral Health
Focus) Schedule: Monday-Friday 8:00am-5:00pm As a Community Health
Worker in our Behavioral Health service line, you’ll play a crucial
role in connecting individuals with mental health and/or substance
use challenges to the support they need to thrive. Through
outreach, advocacy, and education, you’ll bridge the gap between
behavioral health services, social supports, and the
community—helping clients build confidence, navigate complex
systems, and work toward long-term wellness. This is a great

Key Responsibilities Support patients in accessing mental health
and substance use treatment services Provide outreach through home
visits, phone calls, or community-based meetings Educate
individuals and families on wellness, recovery, and behavioral
health risk reduction Coordinate transportation and help manage
appointments and follow-up care Assist with medication adherence
and self-management strategies as directed by clinical staff
Navigate social service systems, including MO HealthNet, DSS, and
local community resources Build relationships with care teams and
community partners to reduce health disparities Conduct PRAPARE
assessments and document patient interactions in the electronic
health record Participate in team meetings and follow care
coordination guidelines as outlined by MO HealthNet Requirements,
Skills, Knowledge and Expertise High School/GED required
Associate’s*/Bachelor’s/Master’s degree in a human service-related
field preferred WORK EXPERIENCE Must qualify for position based on
overall CSS requirements in at least one of the options listed:
Qualified addiction professional Bachelor’s degree in a human
services field from a college or university included in the U.S.
Department of Education’s database of accredited schools:
http://ope.ed.gov/accreditation Any four-year degree or combination
of higher education and at least one year of qualifying experience
Four years of qualifying experience *Associate of Applied Science
in Behavioral Health Support degree as designated by the Department
of Mental Health Qualifying experience must include delivery of
service to individuals with mental illness, substance use
disorders, or developmental disabilities. A valid driver's license
and agency-established minimum automobile coverage required You’ll
