Job Description:
Agape Care Group, a leader in hospice and palliative care in the
Southeast, is seeking a dedicated Registered Nurse for our
After-Hours Weekend Hospice services. In this role, you will be
pivotal in providing compassionate and comprehensive care to
patients during after-hours and weekends, ensuring their comfort
and dignity in their final days. This position requires a
commitment to understanding individual patient needs, effective
communication with family members, and collaboration with a
multidisciplinary team to deliver exceptional care tailored to each
patient’s unique situation. Provide direct nursing care to hospice
patients in after-hours settings. Collaborate with healthcare
professionals to plan, implement, and evaluate patient care plans.
Respond to patient medical needs promptly and proficiently to
ensure comfort and wellbeing. Communicate clearly and
compassionately with patients’ families about care processes and
updates. Administer medications as prescribed and monitor for any
side effects or reactions. Prepare clinical and progress notes for
the health care team. Educate patients and their families on
end-of-life care and self-management techniques. Strong skills in
patient assessment and nursing intervention Ability to work
independently and as part of a multidisciplinary team Excellent
communication and interpersonal skills Proficiency in handling
medical equipment and software Medical Insurance Dental Insurance
Vision Insurance Life Insurance 401k Current Registered Nurse (RN)
license in the state of practice CPR certification Certified
Hospice and Palliative Nurse (CHPN) preferred
