Job Description:

supportive and professionally gratifying environment. Provide
after-hours end-of-life care to patients in various settings.
Assess patient condition during every visit and chart your
observations. Perform evaluation tasks, including vital signs and
medication review. Administer medication as prescribed by the
attending physician. Educate and support patients and their
families on end-of-life care and processes. Maintain a cohesive
communication between health care providers, patients, and their
families to ensure proper management and compassionate care.
Coordinate with physicians and other healthcare professionals to
customize care plans. Strong clinical skills in end-of-life care
Proficient in using healthcare software and systems Adept in
interpersonal communication and patient and family education
Ability to handle emotional and stressful situations Excellent
organizational and time-management skills Medical Insurance Dental
Insurance Vision Insurance 401k Valid Registered Nurse (RN) license
in the state of practice CPR certification Preferred: Certification
in Hospice and Palliative Nursing (CHPN) but not required
